| 内容記述 | 本稿の目的は,特別養護老人ホーム(以下,特養)の終末ケアをめぐる現状について文献を通して概観し,その課題を明らかにすることである。文献検討の結果,以下のことがみえてきた。特養の利用者の介護度分布は重度の方に集中し,死亡退所者が6割以上を占め,終末ケアはより身近である。特養においては,利用者の8割に認知症の症状が認められる。認知症のある高齢者の自己決定をいかに支えていくかを検討することは今後の重要な課題である。特養は本来生活の場であり,「生活」に主眼をおいた終末ケアが求められる。最期までその人らしい生活ができるようサポートするために,具体的にどのような方法で高齢者の願いを汲み取ることができるのかを検討することも重要な課題である。また,特養の終末ケアは看護師が不足する中で行われている。特養における看護のあり方も十分には認識されておらず,終末ケアを可能にするための看護のあり方についても検討していく必要がある。特養では,終末ケアに関する明確なガイドラインが存在しないまま,職員の認識もそれぞれの中で終末ケアが行われている現状も明らかとなった。今後,終末ケアに関心ある職員の資質を生かせるよう終末ケアのガイドラインについても検討していく必要がある。 | |||||

| 内容記述 | The purpose of this study is to clarify through literature the problem of terminal care in the current state of nursing homes. As a result of the literature examination, following fact clarified. In nursing homes, the ratio of elderly people who are in need of great care is increasing. Over 60 % of the causes of leaving nursing homes are by death. This makes terminal care an important question at nursing homes. Among the nursing home residents, about 80 % suffer from dementia. In future, it is an important task to examine the ways to support self decision of the elderly with dementia. A Nursing home is the place of their daily life. Therefore, the terminal care mainly focused on "living" is required. It is also important to search for the means to know what the elderly desire, in order to let them live in their own style to the end of their lives. Then, it is naturally important to examine the ways to fulfill such wishes of the elderly. And, in the nursing home, terminal care is being carried out in the situation of nursing staff shortage. The ways of nursing at nursing homes have not been paid sufficient attention. The role of the nurse in terminal care must also be examined. It became clear that the terminal care is carried out in nursing homes with no clear guideline, and the knowledge and aptitude of the staff also varies great deal. In the future, it will be important to formulate the guideline for terminal care. | |||||
