| 内容記述 | 高齢者の地域での生活を支えるために，現在地域包括ケアシステムが充実されようとしている。2000年にスタートした介護保険制度のもとで要介護認定を受けている高齢者は，2013年で564万人となった。この要介護認定高齢者が地域で生活するために，ケアプランに基づく介護サービスが展開されているが，このケアプランにどのようなサービスが組み入れられるかが，高齢者の生活の質を確保する上で重要となる。ケアプランは主に介護支援専門員によって原案が作成され本人や家族の同意のもと決定される。このケアプランは，介護保険で準備されているサービスと介護保険以外のサービスが組み入れられるため，介護支援専門員が高齢者の住む地域のケアシステムを十分理解し，サービス提供者との連携を密にとり，そのサービスを有効に活用することができるか否かが要介護認定高齢者のQOLを左右するといっても過言ではない。 そこで本稿は，重要な役割を演じる介護支援専門員のネットワーク構築状況を調査することとした。地域包括ケアを実践するための基盤として，ネットワーク構築は欠かせない第一歩ととらえ，その構築状況を明らかにしようとするものである。まず，ネットワーク構築に介護支援専門員同士の特に事業所内のネットワークが影響しているのではないかと仮説を立て検証することを試みた。調査は，介護支援専門員の資格更新の研修会と新人研修会の参加者にアンケート調査を行った。調査内容は介護支援専門員同士の連携，医療機関との連携，多職種との連携，地域包括ケアシステムにおける介護支援専門員の役割の重要性に関する認識等とした。結果は，介護支援専門員同士の事業所内の連携が密であると医療機関との連携がとりやすいことがわかった。また，介護支援専門員は，地域包括ケアシステムで重要な役割を担っていることの認識は十分もっており，ネットワークの構築に関しても重要性を認識できていることがわかった。介護支援専門員同士の連携強化を図ることでネットワーク構築がされやすく，要介護状態となった高齢者のQOLの向上に寄与することが示唆された。 |

| 内容記述 | In order to support elderly in a community, Japan is about to improve the community-based integrated care system. In 2013, 5.64 million of the elderly received care needs certifications under the ling-term care insurance that was implemented in 2000. Those care-needs certified elderly receive long-term care services based on their care plans, so the services included in a care plan are particularly important for them to ensure their quality of life (QoL). First of all, a care manager makes a draft of a care plan and then determines it with the consent by the elderly person or his/her family. This care plan includes services both covered and not covered by the long-term care insurance. Therefore, a care manager needs to understand the care system in the area where the elderly lives, stay in close contact with other care service providers, and utilize the services in a most effective way. These are important factors to ensure their QOL. In this paper, we investigated how well care mangers built the communication network, which plays an important role in the community-based care. For the purpose of this paper, we identified the current situation of the communication network because building a communication network is the first and essential step to implement the community-based care. To start off the investigation, we attempted to verify the hypothesis that building a communication network is influenced most by a network between care managers especially within the same office. We handed out questionnaires at workshops for certification renewal of care mangers and new care mangers. Through the questionnaires, we asked them about the cooperation with other care mangers, hospitals, and interdisciplinary specialists, and the importance of a care manger in the community-based care. We found that the care managers stayed in close with each other within the same office, and that it was easy for them to cooperate with hospitals. Moreover, they fully realized the importance of care mangers in the community-based care and the importance of building a communication network. The study suggests that we can easily establish a communication network through strengthening the cooperation among care managers, and that a strong communication network contributes to improving the QOL of elderly in need of care. |
