{"created":"2023-06-20T14:35:04.119043+00:00","id":505,"links":{},"metadata":{"_buckets":{"deposit":"abe46050-02e0-41bc-aea6-359ea8fa5d35"},"_deposit":{"created_by":15,"id":"505","owners":[15],"pid":{"revision_id":0,"type":"depid","value":"505"},"status":"published"},"_oai":{"id":"oai:kjunshin.repo.nii.ac.jp:00000505","sets":["25:96"]},"author_link":["944"],"item_10002_biblio_info_7":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2020-03-31","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"15","bibliographicPageEnd":"52","bibliographicPageStart":"43","bibliographic_titles":[{"bibliographic_title":"鹿児島純心女子大学大学院人間科学研究科紀要"}]}]},"item_10002_description_5":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"近年,不登校の問題が長期化・深刻化しており,臨床心理職を含めた多職種専門家による訪問型の支援に期待が高まっている。本稿は,不登校状態にある発達障害児への訪問支援の中で,再アセスメントのために心理検査(WISC― Ⅳ)を実施し,他職種,他機関と連携,協働し,包括的な支援を目指したプロセスを踏まえ,訪問支援における臨床心理職の役割について,特に,心理検査によるアセスメントに注目して検討することを目的した。対象は,幼児期に自開スペクトラム症と注意欠如・多動症の診断を受け,入退院を繰り返し,不登校が長期化している小学6年生女児Aである。退院後に導入となった,臨床心理職を含めた多職種チームでの訪問支援の中で,復学へ向けた再アセスメントを目的とした心理検査(WISC― Ⅳ)を実施し,入院中に実施したものと比較検討した。その結果,入院中と退院後の双方で【処理速度】が有意に低く,発達障害児の傾向に合致した。また,入院中の結果と比較して全体的に評価点が下がり,特に学習活動と強く関連している【言語理解】の落ち込みが日立ち,下位検査の中では〔単語〕と〔算数〕が低値であった。学校教育から長期間離れていることが大きく影響し,年齢相応の結晶性知識の未獲得が示唆されるが,抽象的カテゴリー化を必要とする〔類似〕の高値から,高い言語能力が備わっていることが窺える。行動観察では,入院中の実施時とは一変して,表情が柔らかく,意欲的,積極的な姿が見られた。心理検査の結果を踏まえ,他職種,他機関と連携し,支援方針や具体的支援方法の再検討を行い協働体制を整えた。訪問場面での心理検査について,結果の妥当性の問題や環境設定の難しさなど課題も多いが,研究報告は希少であるため,一事例ではあるが個々のニーズに沿った,より効果的実際的なアセスメントを目指した本研究の意義は大きいと考えられる。","subitem_description_type":"Abstract"},{"subitem_description":"Truancy is a serious, long-term problem that requires home-visit-support provided by multidisciplinary professionals. A psychological test was conducted to reassess a truant child with developmental disorders during home-visit-support provided by a clinical psychologist with the collaboration of a multidisciplinary team. The role of clinical psychologists in providing home-visit-support was examined by focusing on the child's assessment.\nThe client was a girl in the sixth grade of elementary’ school that had been diagnosed with ASD and attention deficit hyperactivity disorders in early childhood. She had experienced repeated hospitalization and discharge and had not been attending school for a long time.\nAfter the discharge, home-visit-support was provided by a professional team, including a clinical psychologist that reassessed the child using WISC-IV, and compared the results with that during hospitalization.\nThe child's \"processing speed\" was significantly low during hospitalization and after discharge. The evaluation scores generally declined after discharge. Notably, “language understanding\" as well as \"words\" and \"mathematics\" declined considerably. Long-term leave from school might have affected the above results, and crystallized intelligence consistent with age was not acquired. On the other hand, the child had a high \"similarities\" score, which requires abstract categorization, suggesting a high linguistic ability. The behavioral observation indicated a motivated and proactive attitude after discharge, which was opposite to during hospitalization. Based on the results, support policies and concrete support measures were reexamined through inter-professional cooperating, and a cooperative support program was developed.\nThere are some problems in conducting psychological tests during home-visits, such as problems of validity and the testing environment.
